How do I apply
for DPS with your Company? |

To apply, you can: |

a) |
Complete the Proposal Form

here to download the form) and mail it to us,
or |
b) |
Get in touch with your Life
Planner, or |
c) |
Drop by our Customer Care Centre
at 1 Pickering Street, #01-01, Great Eastern Centre,
Singapore 048659. |

What happens after I have submitted
my Proposal Form to your Company? |

Our Underwriter will evaluate your
application and a decision will be issued to you 5 - 7 working
days after receiving your Proposal Form.

What is my premium and how
do I make premium payments? |
|
The premium payable depends on your
age (please refer to the premium table in the Product Summary).

When your application has been accepted by the Company,
we will request for the premium from your CPF Ordinary Account
/Special Account.

What is the amount of my coverage
and what am I covered for? |
|
The coverage amount is $46,000. The
policy covers death and permanent incapacity, which under
DPS is defined as being physically or mentally incapacitated
from ever continuing in any employment.

Do I need to complete any Financial
Needs Analysis when I apply for DPS? |

Form. You may select the desired options of your choice. |
