Submit A Claim
Follow these 3 easy steps to submit a claim:
1.
Select and print the required claim form from the list of claim forms
2.
Complete the required forms and ask your doctor to complete the Doctor's Statement.
3.
Submit all the required forms and supporting documents to Claims Department for processing.
